What is Health Care Transition?
Health care transition, or HCT, is the process of moving from a child/family-centered model of health care to an adult/patient-centered model of health care, with or without transferring to a new clinician. It involves planning, transfer, and integration into adult-centered health care.

Lesley University Threshold Program Virtual Open House for Diverse Learners
Virtual Information Session, April 5, 2021, 4 – 6 p.m., 2020, with Lesley University Threshold Program in Cambridge, MA. This virtual open house will offer young adults with diverse learning differences and their caregivers and families a chance to get firsthand information about the Threshold Program from faculty, the admissions team, and directly from alumni.

MassHealth’s Adult Family Care & PCA programs for Adult Children with Disabilities
Webinar, April 13, 2021, 7 p.m, with the Special Needs Transition Group of Wakefield, MA.
* MassHealth’s Adult Family Care (AFC) and PCA programs both help pay for the caregiving of qualified adult children (including care provided by the child’s own parents, if they are not the guardians).
Speakers Beth Gutierrez, Director of the Adult Family Care Program for Advocates, Inc., and Linda Andrade, Director of Community Services at Tempus Unlimited, will explain the two programs, their benefits, and eligibility requirements.

The Achieving a Better Life Experience (ABLE) Attainable Savings Plan
Webinar April 13th, 2021, with the Massachusetts Educational Financing Authority (MEFA). The Attainable Savings Plan provides flexibility, tax benefits, and independence to those individuals with disabilities who save in the program.
  • The Attainable Savings Plan allows individuals with disabilities and their families to save for disability-related expenses without losing eligibility for federal means-tested benefits.
  • In this webinar, review the benefits, eligibility requirements, and plan details of Attainable, and explain how to easily set up an account.

About the Speaker: Mary Rubenis is the Attainable Outreach Manager for the Massachusetts ABLE Savings Plan, offered by the Massachusetts Educational Financing Authority (MEFA). Mary assists families and school professionals with college planning, financial aid and tax-advantaged savings plans.

What is TOPSoccer?
TOPSoccer is a modified version of the game of soccer, designed to provide opportunities for children who have challenges that otherwise prevent them from participating in a traditional soccer program. Our program invites all athletes with special needs to develop both physically and emotionally, while providing opportunities for fun, fitness, and building self-esteem.
High school soccer players participate in the TOPS program as ‘Buddies.’ A Buddy is partnered with each player providing one-on-one assistance and enabling the player to participate at their desired level, using a recreational, leisurely approach to the game. Buddies facilitate modifications to the game to allow full participation and enjoyment for each child.
North Reading Youth Soccer is devoted to providing a constructive and accommodating soccer experience for children that have challenges – physical, cognitive, or otherwise – through TOPSoccer. Our program has enjoyed great support from the community, the players, and their parents since its inception in 2015. TOPS players join us from all over the region, including North Reading, Reading, Andover, North Andover, Boxford, Burlington, Danvers, Lynnfield, Stoneham, Lawrence, and Derry, NH!

  • If your athlete has a challenge that prevents involvement on traditional soccer teams
  • If you and your athlete want a safe environment to enjoy the game of soccer
  • If your athlete is age 4 through 19
Spring 2021 Details
  • Sessions take place on Saturdays from 4:00 – 5:00pm at Ipswich River Park, North Reading
  • The Spring 2021 season runs from April 24 – June 12 (excluding Memorial Day Weekend)
  • Registration is $25 per player and includes a shirt/ soccer ball for new players, and a celebration with a trophy/medal at season’s end
Volunteers Needed
  • TOPS welcomes new volunteers to be Buddies and Coaches. Prior soccer experience not required
  • Buddies earn Community Service hours
  • Enjoy incredible experiences working with great kids!
